Save or print
Use the Print or save as PDF button at the bottom of each form. On a phone, choose Save to Files or Save as PDF from the print screen.
Bring it to your visit, or fax it
Bring the printed or saved forms to your first appointment, or fax them ahead to (936) 270-8012. Please don't email completed forms, since regular email isn't a secure way to send health information.
What to bring to your first visit
- Your completed forms (or arrive 15 minutes early to fill them out)
- Insurance card and a Texas ID or driver's license
- A list of current medications and allergies
- Recent lab results if you have them (CBC, CMP, TSH, T4, liver function, lipid panel)
- For a child or teen, a parent or legal guardian
